judging by the absolute emotional wreck of Foggy after the mysterious object he finds in the drawer of Matts office my guess is that it's:

the suicide revolver from smiths issue #6

it's not only a failure on Matts part to find positive solutions rather than denial to fix his psyche but also insulting to Foggys friendship and loyalty even after Shadowland that Matt would be this stupid and selfish to consider the option.

There's tons of other possibilities but this seems the most logical (although cliche' at this point in Matts story) as Foggy has never seen or faced the issue of a Matt Murdock on the verge of suicide due to manic depression.

I kinda thought it might be an engagement ring. It would be disappointing if it was but that's what I first thought.

If it is connected to Matt's new found happiness maybe it's valium and we'll get a story with DD dealing with drug addiction.

Or whatever was in the suitcase from Pulp Fiction.

I like what Waid's doing with DD right now, but unless it's something totally unexpected I cannot imagin anything that would have Foggy _again_ deciding to abandon Matt and at the same time make sense.
Suppose it's the skull of matt's dad.

"Yeah, I took the skull with me. So what. Moleman had destroyed the whole graveyard and I needed something to remind me of my that. How is that worse than kicking bad guys in the nuts?"

"Yeah, some anti-depression pills. So what. I've been literally through hell. You want to abandon me because the cat I'm taking medicines scares you?"
